Course Reserve Request Form

If you have not submitted your course reserve requests through Chalk, submit them using the following form.

If you have more than five items to place on reserve for a single course/seminar, you may submit the form multiple times, but please make a note of the multiple submissions in the Comments field (for example, "This is form submission 1 of 2 for LAWS 40501").

If the fields provided do not to apply to an item that you would like to request, please use the Comments field at the end of the form to describe the item as fully as possible.

Do not include the books that you requested on your textbook order form. These are placed on reserve automatically following your textbook order submission.
